If you live with hypermobile Ehlers-Danlos syndrome (hEDS), you already know the daily juggling act: joints that move further than they should, unpredictable pain, a body that seems to "give way" at the worst moments, and often a racing heart or dizziness when you stand up too quickly. Many people in the hEDS community have started reaching for compression tights and leggings as part of how they manage these symptoms, and in the last few years, researchers have finally started catching up with what patients have been saying for a long time. Below is a look at what the science actually shows.

Compression and Joint Stability

Hypermobile EDS affects the connective tissue that normally keeps joints, tendons, and ligaments working together in a coordinated way. One consequence is impaired proprioception, the sense that tells your brain where your joints are in space without you having to look. When that sense is unreliable, joints are more prone to buckling, subluxing, or simply feeling unstable during everyday movement.

A randomized controlled trial published in Disability and Rehabilitation in 2023 tested this directly. Thirty-two adults with hEDS were assigned to either physiotherapy alone or physiotherapy combined with compression garments. After four weeks, the compression group showed slower, smaller postural sway and a notably better Romberg quotient (a standard measure of how much the body relies on vision to stay balanced), especially when standing on an unstable surface. The researchers concluded that compression garments "could compensate for proprioceptive impairment," giving the nervous system extra sensory information to work with when the joints themselves aren't providing reliable feedback. A separate prospective cohort study out of the Raymond Poincaré Hospital's EDS reference center followed 67 people with non-vascular forms of EDS over two years and found a similar pattern: pain at the most affected joint dropped from an average of 71.5 to 45.7 on a 100-point scale, while ankle sprains, knee sprains, and shoulder and knee subluxations all fell significantly over the study period.

Pain Relief

Pain reduction is one of the most consistently reported benefits, both in research and in patient communities. A retrospective cohort study out of Amiens University Medical Center followed 20 adults with hEDS or hypermobility spectrum disorder who wore compression garments (fitted at roughly 90-95% compression on the limbs and trunk) for an average of 8.3 hours a day. Eighty percent of participants reported reduced pain, just over half were able to cut back on pain medication, and nearly half described a rapid "on-off" effect, with pain easing noticeably soon after putting the garment on. The same study's authors were careful to note that most participants were also doing physiotherapy alongside compression use, so the pain relief can't be attributed to compression alone. Still, it's a meaningful signal, and it lines up with the balance study above, which found joint pain scores dropped more than twice as much in the compression group as in the physiotherapy-only group.

Choosing a Compression Level

Compression strength is measured in millimeters of mercury (mmHg), and different levels suit different needs. Mild symptoms, general joint support, and everyday wear are typically well served by 15-20 mmHg.
